Important Information:

Invisalign Treatment Agreement

The undersigned patient acknowledges that the Invisalign treatment provided under this agreement will be fully covered by OFFDENTAL, subject to the following conditions:

Invisalign Treatment Costs
All costs directly related to the Invisalign treatment, including the planning, trays, adjustments, and follow-up visits, will be fully covered by OFFDENTAL.
Exclusions
This giveaway does not cover other dental services, such as cavities, cleanings, implants, crowns, veneers, or any other restorative or cosmetic treatments. These services will be billed to the patient's insurance (if applicable) or must be paid out-of-pocket.
Valid Time frame
The Invisalign treatment offer is valid for 30 days from the date the winner is informed of their selection. Failure to begin treatment within this period will result in the offer being revoked and considered void.

Eligibility and Terms

Age Requirement: Participants must be 18 years or older.
Location: Open to residents of New York, New Jersey, and Connecticut.
Patient Status: Open to new and existing patients.
Duration: The giveaway runs until June 15, with one winner announced each month.
